Copyright notices must identify the work, identify the material and its location precisely enough for us to find it, include your contact details, state a good-faith belief that the use is unauthorised, state that the notice is accurate under penalty of perjury, and be signed.

How to send a copyright notice, and how a customer sends a counter-notice.
We forward valid notices to the customer and give them an opportunity to remove the material or respond. Where the material is clearly infringing and the customer does not act, we remove or disable access to it.
A customer who believes material was removed in error may send a counter-notice. Where a valid counter-notice is received, we restore the material within the statutory window unless the complainant notifies us of legal action.
Accounts that accumulate repeated valid infringement notices are terminated.
